This form can be used to order DMAP products which are to be paid for by Cheque, Money Order, Bank Transfer, or Official Order. The product(s) will be sent to you immediately on receipt of the payment or official order. Cheques and Money Orders should be made payable to 'DMAP'. For Bank Transfer details, Contact Alan Morton.
